The collection of interventions in the Connecting to Care workbooks can be utilized as a tool in developing new agency programs or rethinking and re-designing current programs to better engage clients currently not in regular medical care. Please keep in mind that each intervention may be implemented as described here, be adjusted to better meet agency needs, or can provide useful elements that may enhance existing agency programs. Additionally, we encourage you to consider reviewing a variety of approaches: individual, group and community-level interventions as each can provide new ideas for familiar programs. |
